Application Checklist
- Is your application signed and complete with full names, dates of birth, social security numbers, etc.?
- Did you provide the following:
- Copy of each family member's birth certificate.
- Copy of each family member's social security card.
- Copy of each adult member's driver's license or photo ID card.
- Copy of most recent bank statement if applicable.

Do you pay for child care while working? If yes, provide written verification of payment for the last three months from the child care provider.
If applying for HiRise:
Do you pay out-of-pocket for medical expenses? If yes, provide written verification of payment within the last 12 months.
